Parent(s) and Legal Guardian(s) can self-refer by calling our referral line at (905) 568-3806, extension 3203. Parents are encouarged to leave a voice message with their name and telephone number, and a team member will call back to ask further questions about their child.

Professionals:
With parental consent, Physicians and Professionals can refer by completing the ICDSP Referral Form available below. Completed referral forms can be faxed to the attention of Intake at 905-568-9295
